Completion of Form G-120, Transcript Designation and Ordering Form

  • Complete items 1 through 20 on the Transcript Designation and Ordering Form G-120, {see form G-120}
    (Complete only one Transcript Designation and Ordering Form for all court reporters on the case)
    (Complete only one Transcript Designation and Ordering Form for all digitally recorded proceedings/Court Smart hearings)
  • For specific instructions on filling out the Transcript Designation and Ordering Form, {see form instructions; G-120A}
  • Log in to your CM/ECF account.
  • Electronically file the Transcript Designation and Ordering Form with the appropriate attachments; for example, CJA 24 voucher; letter of authorization, if appropriate; Court Order for sealed proceedings.

Completion of CJA 24 Voucher

  • Box 13. It is acceptable to indicate “see TDO” in box 13 of the CJA 24 voucher or submit one CJA 24 voucher per court reporter/recorder with his or her corresponding dates.
  • Box 15. Include attorney’s signature, electronic or digital signature, printed name and date of signature.  If box 15 is incomplete, a Notice of Deficiency will be issued.  
  • Box 14. If requesting Special Authorizations in box 14 of the CJA 24 Voucher, attach a letter addressed to the assigned District Court Judge justifying the requests as an attachment to Form G-120. If requesting transcripts for purposes other than a Direct Appeal, you must attach a letter addressed to the assigned District Court Judge requesting CJA payment per box 16 of the CJA 24.
  • Box 22. Do not sign this box unless instructed by Court Reporting Services staff member.

CM/ECF Filing of Transcript Designation and Ordering Form G-120

  • Log in to your CM/ECF account.
  • Click on Criminal > Other Filings > Transcripts.
  • Enter the district court case number.
  • Select the appropriate case.
  • Select the following event:  Transcript Designation and Ordering Form (G-120).
  • If requesting transcripts from court reporters only, select the radio button that says “Court Reporter.”  
    If requesting transcripts from court recorders/Court Smart hearings, select the radio button that says, “Court Smart – Digitally Recorded Proceedings.”
    If requesting combined transcripts – court reporters and Court Smart hearings – submit separate Transcript Designation and Ordering Forms, one for court reporters and one for Court Smart hearings. 
  • Appeal or non appeal - select appropriate radio button.
    If appeal, enter Court of Appeal case number; next screen, select Criminal Justice Act.
    If non appeal, select Criminal Justice Act.
  • Pay close attention to the next reminder screen in red.
  • Enter all dates of transcripts requested on the Transcript Designation and Ordering Form.
  • Attach the PDF of the Transcript Designation and Ordering Form with all appropriate attachments; CJA 24 voucher, letter of authorization, Court Order for sealed transcripts.
